If you would like to submit classroom pictures to be considered for posting on the Space Explorers website, we ask that this form be completed and submitted to the Public Affairs Officer (fax to 920-339-4619) along with the photo(s) and/or other material you will be submitting.

To be completed by parent/legal guardian

If you submit a unique photograph of your student(s) participating in a Space Explorers program, Space Explorers may request parental/guardian permission to use the image for promotional purposes.
